Summary
Methylphenidate may modestly reduce the clearance of tricyclic antidepressants, which can raise nortriptyline concentrations. This may increase anticholinergic effects, cardiovascular effects and the risk of toxicity. Both drugs can also raise heart rate and lower seizure threshold.
What this interaction means in plain words
Methylphenidate transdermal and Nortriptyline can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Methylphenidate may inhibit hepatic metabolism of tricyclic antidepressants, with additive sympathomimetic and pro-convulsant effects.
What to do
Monitor heart rate, blood pressure and signs of tricyclic toxicity, and consider checking nortriptyline levels. Reduce the nortriptyline dose if toxicity appears, especially when methylphenidate is started or stopped.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
