Summary
Dofetilide is a potent QT-prolonging antiarrhythmic, and nortriptyline can prolong the QT interval too. Combined use increases the risk of torsades de pointes.
What this interaction means in plain words
Dofetilide and Nortriptyline are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Dofetilide belongs to Cardiovascular; Nortriptyline belongs to Central Nervous System.
Why it happens
Additive prolongation of cardiac repolarization via potassium channel effects.
What to do
Avoid the combination and choose a non-QT-prolonging antidepressant. Dofetilide is initiated with inpatient ECG monitoring, so review all co-medications before starting.
