Summary
ACE inhibitors reduce aldosterone-mediated potassium excretion, and intravenous potassium phosphates add a direct potassium load. Together they can produce clinically significant hyperkalemia with risk of cardiac arrhythmia, especially in renal impairment.
What this interaction means in plain words
Moexipril and Potassium phosphates can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Additive effect: ACE inhibition decreases aldosterone-dependent renal potassium excretion while potassium phosphates supply exogenous potassium.
What to do
Avoid if possible; if IV potassium phosphates are required, monitor serum potassium and renal function closely and reduce the infusion rate or dose as needed.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
