Summary

Both drugs lower blood pressure, and starting an ACE inhibitor in a patient taking an alpha-blocker such as prazosin can cause an exaggerated drop in blood pressure. Symptomatic hypotension, dizziness and syncope are most likely after the first dose or after a dose increase.

What this interaction means in plain words

Moexipril and Prazosin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Moexipril belongs to Cardiovascular; Prazosin belongs to Genitourinary.

Evidence level: Moderate

Why it happens

Additive pharmacodynamic vasodilatory and blood pressure lowering effects of ACE inhibition and alpha-1 blockade.

What to do

Start the added agent at a low dose, preferably at bedtime, and monitor blood pressure and orthostatic symptoms after initiation and dose changes. Consider reducing the prazosin dose or holding diuretics when starting moexipril.
