Summary
Selegiline patch labeling contraindicates opioids such as meperidine, tramadol and methadone, and advises caution with other opioids because of rare serotonin syndrome or severe reactions. Morphine has weak serotonergic activity so the risk is lower but cannot be excluded.
What this interaction means in plain words
Morphine and Selegiline transdermal can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
MAO inhibition may potentiate opioid toxicity through serotonergic or other poorly characterized mechanisms.
What to do
Avoid if possible; if morphine is required, use low doses and monitor for hypotension, hyperthermia, agitation and excessive sedation. Choose a non-serotonergic analgesic where practical.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
