Summary

Nafcillin is a CYP3A4 inducer and may lower siponimod exposure, which could reduce its effect on multiple sclerosis disease control. The effect is greater in patients with reduced CYP2C9 activity, who rely more on CYP3A4 for siponimod clearance.

What this interaction means in plain words

Nafcillin and Siponimod can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Induction of CYP3A4 increases siponimod metabolism and lowers plasma concentrations.

What to do

Avoid combining with siponimod in patients with CYP2C9*1/*3 or *2/*3 genotypes. In other patients, consider an alternative antibiotic or monitor for loss of efficacy during a prolonged course.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
