Summary

NSAIDs can lower renal function and reduce clearance of aminoglycosides, with the best documented effect in preterm neonates given indomethacin or ibuprofen. Oral neomycin is poorly absorbed, so systemic exposure is small in most patients and a meaningful interaction with naproxen is unlikely.

What this interaction means in plain words

Naproxen and Neomycin po have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Low

Why it happens

NSAID-induced reduction in renal prostaglandin-dependent perfusion and glomerular filtration could slow clearance of any absorbed aminoglycoside.

What to do

No routine action is needed with intact gut and normal renal function. Monitor renal function if neomycin is given for a long period or with gut mucosal damage or renal impairment.
