Summary
Naproxen can blunt the blood pressure lowering effect of telmisartan. In combination with an angiotensin receptor blocker, NSAIDs can also reduce glomerular filtration and cause acute kidney injury and hyperkalemia, especially in elderly or volume-depleted patients.
What this interaction means in plain words
Naproxen and Telmisartan-amlodipine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Naproxen belongs to Musculoskeletal; Telmisartan-amlodipine belongs to Cardiovascular.
Why it happens
NSAID inhibition of vasodilatory renal prostaglandins reduces renal perfusion and sodium excretion, opposing ARB effects and adding to the risk of renal impairment and hyperkalemia.
What to do
Monitor blood pressure, serum creatinine and potassium when starting or stopping naproxen, and keep the NSAID course short at the lowest effective dose. Ensure adequate hydration and consider paracetamol or another alternative in high-risk patients.
What to ask your pharmacist
