Summary
NSAIDs can reduce the antihypertensive effect of angiotensin receptor blockers and, in volume-depleted or elderly patients, may precipitate acute kidney injury. Hyperkalemia can also occur.
What this interaction means in plain words
Naproxen and Telmisartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Naproxen belongs to Musculoskeletal; Telmisartan belongs to Cardiovascular.
Why it happens
NSAIDs inhibit vasodilatory renal prostaglandins, reducing glomerular perfusion that is dependent on them while ARB blocks efferent arteriolar tone, impairing filtration and sodium excretion.
What to do
Maintain hydration and monitor blood pressure, serum creatinine and potassium after starting or changing naproxen. Avoid or limit NSAID use in elderly, dehydrated or renally impaired patients.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
