Summary

Sulfamethoxazole inhibits CYP2C9 and can increase exposure to CYP2C9 substrates such as nateglinide. This may raise the risk of hypoglycemia, and sulfonamides can also have hypoglycemic effects of their own.

What this interaction means in plain words

Nateglinide and Sulfamethoxazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Inhibition of CYP2C9-mediated nateglinide metabolism, with possible protein binding displacement.

What to do

Monitor blood glucose during co-trimoxazole therapy and reduce the nateglinide dose if hypoglycemia develops.
