Nefazodone inhibits serotonin reuptake and antagonizes 5-HT2A receptors, and combining it with an MAO inhibitor may lead to serotonin syndrome. Labeling for nefazodone contraindicates MAOIs.

What this interaction means in plain words

Nefazodone and Selegiline are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: Moderate

Why it happens

Additive serotonergic effects from reuptake inhibition combined with MAO inhibition.

What to do

Do not combine. Allow a washout of at least 14 days after stopping selegiline before nefazodone, and 7 days after stopping nefazodone before selegiline.
