Summary

Neostigmine increases vagal tone and slows heart rate, and beta-blockers such as penbutolol also reduce heart rate and AV conduction. Together they may produce additive bradycardia or AV block, particularly in patients with existing conduction disease.

What this interaction means in plain words

Neostigmine and Penbutolol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Additive pharmacodynamic effects on heart rate and AV conduction through cholinesterase inhibition and beta-adrenergic blockade.

What to do

Monitor heart rate and ECG when neostigmine is given to patients taking penbutolol. Have atropine or glycopyrrolate available to treat symptomatic bradycardia.

What to ask your pharmacist

  • Should I take them at different times of day?
