Summary
Neostigmine inhibits plasma cholinesterase as well as acetylcholinesterase, which slows succinylcholine hydrolysis and can prolong or intensify its blockade. Neostigmine may also change a phase I depolarizing block into a prolonged phase II block. This can delay recovery of spontaneous breathing.
What this interaction means in plain words
Neostigmine and Succinylcholine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Neostigmine belongs to Central Nervous System; Succinylcholine belongs to Musculoskeletal.
Why it happens
Cholinesterase inhibition reduces succinylcholine metabolism and increases acetylcholine at the neuromuscular junction.
What to do
Avoid giving neostigmine before the succinylcholine block has resolved, and use nerve stimulation to confirm recovery. Be prepared to provide ventilatory support if blockade is prolonged.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
