Summary

Procainamide has weak neuromuscular blocking and anticholinergic-like properties that can oppose the effect of neostigmine. In patients with myasthenia gravis this may worsen muscle weakness and reduce the benefit of the anticholinesterase.

What this interaction means in plain words

Neostigmine and Procainamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Pharmacodynamic antagonism, as procainamide impairs neuromuscular transmission and has antimuscarinic activity.

What to do

Monitor muscle strength in patients with myasthenia gravis who receive procainamide and adjust the neostigmine dose if needed. Prefer a different antiarrhythmic where practical.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
