Summary

Protriptyline has strong anticholinergic activity that can oppose the cholinergic effects of neostigmine. This may reduce the benefit of neostigmine in myasthenia gravis or in reversal of neuromuscular blockade, and can blunt its effect on gut and bladder motility.

What this interaction means in plain words

Neostigmine and Protriptyline can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Pharmacodynamic antagonism between a cholinesterase inhibitor and a tricyclic antidepressant with muscarinic receptor blocking activity.

What to do

Monitor the clinical response to neostigmine, such as muscle strength or bowel and bladder function, and adjust the neostigmine dose if needed. Consider a less anticholinergic antidepressant if the effect is clinically important.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
