Summary

Neostigmine reverses the effect of nondepolarizing neuromuscular blockers like rapacuronium by increasing acetylcholine at the neuromuscular junction. This is the intended basis of reversal, but the interaction is not a harmful one in that context. Rapacuronium is no longer marketed because of fatal bronchospasm.

What this interaction means in plain words

Neostigmine and Rapacuronium have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Moderate

Why it happens

Cholinesterase inhibition by neostigmine increases acetylcholine, which competitively overcomes the nicotinic receptor blockade produced by rapacuronium.

What to do

This is an expected pharmacologic reversal. Confirm recovery of neuromuscular function with train-of-four monitoring before extubation, and give an antimuscarinic such as glycopyrrolate to limit cholinergic side effects.
