Summary

Tricyclics can potentiate the pressor response to direct-acting sympathomimetics by blocking norepinephrine reuptake. Oral phenylephrine decongestants may therefore produce greater blood pressure elevation or arrhythmia in patients taking nortriptyline.

What this interaction means in plain words

Nortriptyline and Phenylephrine po can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Inhibition of norepinephrine reuptake by the tricyclic increases adrenergic receptor stimulation.

What to do

Use the lowest decongestant dose for the shortest time and monitor blood pressure; consider a non-pressor alternative such as saline or intranasal steroid.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
