Summary
Tolvaptan is a vasopressin antagonist that causes aquaresis and has been associated with increases in serum potassium. Together with an angiotensin receptor blocker, which also reduces potassium excretion, hyperkalemia may be more likely. Rapid fluid loss may also add to hypotension.
What this interaction means in plain words
Olmesartan and Tolvaptan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Olmesartan belongs to Cardiovascular; Tolvaptan belongs to Endocrine.
Why it happens
Additive potassium retention from ARB-mediated aldosterone reduction and tolvaptan effects, with additive volume depletion and hypotension.
What to do
Monitor serum potassium, renal function and blood pressure after starting tolvaptan.
