Summary

Parecoxib, a COX-2 inhibitor prodrug, can reduce renal prostaglandin synthesis, causing sodium and potassium retention and reduced diuretic response. With spironolactone this raises the risk of hyperkalemia and acute kidney injury.

What this interaction means in plain words

Parecoxib and Spironolactone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

COX-2 inhibition reduces renal perfusion and prostaglandin-mediated potassium excretion, adding to aldosterone antagonism.

What to do

Monitor serum potassium and creatinine, ensure adequate hydration, and use the shortest effective parecoxib course.

What to ask your pharmacist
