Summary
Parecoxib is converted to the COX-2 inhibitor valdecoxib and can reduce the antihypertensive effect of trandolapril. In dehydrated, elderly or renally impaired patients the combination may cause acute kidney injury and hyperkalemia.
What this interaction means in plain words
Parecoxib and Trandolapril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
COX-2 inhibition reduces renal prostaglandin synthesis, opposing ACE inhibitor effects and lowering renal perfusion.
What to do
Monitor blood pressure, renal function and potassium, maintain hydration, and limit parecoxib to the shortest duration.
What to ask your pharmacist
