Summary
Parecoxib, converted to the COX-2 inhibitor valdecoxib, can impair renal prostaglandin function. With triamterene this increases the risk of hyperkalemia and acute kidney injury and may blunt diuretic efficacy.
What this interaction means in plain words
Parecoxib and Triamterene can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
COX-2 inhibition reduces renal perfusion and renin-aldosterone activity, adding to triamterene's potassium retention.
What to do
Avoid in renal impairment, heart failure or dehydration. If combined, monitor serum potassium and creatinine and keep the NSAID course short.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
