Summary

NSAIDs such as parecoxib can reduce renal prostaglandin synthesis, which may blunt the antihypertensive effect of valsartan and impair renal function. They can also contribute to hyperkalemia, particularly in volume-depleted, elderly or renally impaired patients.

What this interaction means in plain words

Parecoxib and Valsartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: High

Why it happens

COX-2 inhibition reduces vasodilatory renal prostaglandins, causing sodium retention and reduced glomerular perfusion, with reduced aldosterone and potassium excretion when combined with ARB.

What to do

Ensure adequate hydration and monitor renal function, potassium and blood pressure, especially within the first weeks. Use the lowest effective parecoxib dose for the shortest time.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
