Summary
Non-selective beta-blockers can modestly raise serum potassium by impairing cellular potassium uptake. With triamterene, hyperkalemia risk increases slightly, mainly in patients with renal impairment or diabetes. Additive blood-pressure lowering also occurs.
What this interaction means in plain words
Penbutolol and Triamterene have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Why it happens
Beta-blockade reduces beta-2 mediated cellular potassium uptake and renin release, adding to triamterene potassium retention.
What to do
Monitor serum potassium in patients with renal impairment or diabetes; routine action is otherwise unnecessary.
What to ask your pharmacist
