Summary
Physostigmine inhibits acetylcholinesterase and raises acetylcholine, whereas scopolamine blocks muscarinic receptors, so each opposes the other's effect. Physostigmine is used deliberately to reverse anticholinergic toxicity from scopolamine, which can reduce or abolish scopolamine's intended effects.
What this interaction means in plain words
Physostigmine and Scopolamine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Pharmacodynamic antagonism between a cholinesterase inhibitor and a muscarinic antagonist.
What to do
Avoid giving them together unless reversing scopolamine toxicity; in that case titrate physostigmine slowly with cardiac monitoring because of bradycardia and seizure risk.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
