Summary

Sacubitril inhibits OATP1B1 and OATP1B3 in vitro, and sacubitril-valsartan has been shown to raise exposure to some OATP substrate statins such as atorvastatin. Higher statin levels could increase the risk of muscle toxicity, though the effect on pitavastatin is not well characterized.

What this interaction means in plain words

Pitavastatin and Sacubitril-valsartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Pitavastatin belongs to Cardiovascular; Sacubitril-valsartan belongs to Cardiovascular.

Evidence level: Low

Why it happens

Inhibition of hepatic OATP1B1/1B3 uptake transporters by sacubitril reduces statin hepatic uptake and raises plasma concentrations.

What to do

Monitor for muscle pain, weakness or dark urine and check creatine kinase if symptoms occur. Use the lowest effective pitavastatin dose.

What to ask your pharmacist
