Summary

NSAIDs such as tolmetin can blunt the blood pressure lowering effect of alpha-blockers like prazosin. Sodium and fluid retention together with reduced vasodilatory prostaglandins may lead to loss of blood pressure control.

What this interaction means in plain words

Prazosin and Tolmetin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Prazosin belongs to Genitourinary; Tolmetin belongs to Musculoskeletal.

Evidence level: Low

Why it happens

NSAID inhibition of prostaglandin synthesis causes sodium and water retention and reduces vasodilation, opposing the antihypertensive effect.

What to do

Monitor blood pressure when tolmetin is started, changed or stopped in a patient taking prazosin. Adjust the antihypertensive dose if control worsens, or use a non-NSAID analgesic.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
