Summary

Trospium and procainamide are both eliminated partly by active tubular secretion in the kidney. Competition for this pathway may raise serum concentrations of either drug, with a risk of increased procainamide toxicity such as QRS or QT prolongation and arrhythmia, or greater anticholinergic effects from trospium.

What this interaction means in plain words

Procainamide and Trospium can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Competition for renal tubular secretion via cationic transport pathways reduces renal clearance of both drugs.

What to do

Monitor ECG and procainamide (and NAPA) plasma concentrations when the combination is used. Watch for anticholinergic adverse effects and reduce doses if renal function is impaired.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?

How serious is the interaction between Procainamide and Trospium?

MediHelp rates it moderate, on a four-level scale (minor, moderate, major, contraindicated) based on published labels and references. What to do, in brief: Monitor ECG and procainamide (and NAPA) plasma concentrations when the combination is used. Watch for anticholinergic adverse effects and reduce doses if renal function is impaired.

What other medicines interact with Trospium?

MediHelp also lists interactions of Trospium with, for example, Clozapine, Pramlintide, Thioridazine. The full list is on the Trospium page.
