Summary
Pyridostigmine, a cholinesterase inhibitor, opposes the effect of non-depolarizing blockers such as rocuronium, so patients taking it (e.g. for myasthenia gravis) may show resistance to or unpredictable response to rocuronium. Myasthenic patients can also be unusually sensitive to the blocker, making the response hard to predict.
What this interaction means in plain words
Pyridostigmine and Rocuronium can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Pyridostigmine belongs to Central Nervous System; Rocuronium belongs to Musculoskeletal.
Why it happens
Increased synaptic acetylcholine from cholinesterase inhibition competes with rocuronium at nicotinic receptors.
What to do
Titrate rocuronium cautiously using quantitative neuromuscular monitoring, starting with a reduced dose in myasthenia gravis. Plan for postoperative ventilatory support if needed.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
