Summary
In patients with myasthenia gravis, starting a corticosteroid can cause a transient worsening of muscle weakness, and this may be more pronounced in patients already taking an anticholinesterase agent. Respiratory and bulbar weakness can occur in severe cases.
What this interaction means in plain words
Pyridostigmine and Triamcinolone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Pyridostigmine belongs to Central Nervous System; Triamcinolone belongs to Endocrine.
Why it happens
Corticosteroids can cause an early, transient exacerbation of myasthenic weakness; the exact mechanism of the interaction with cholinesterase inhibitors is not well established.
What to do
Monitor myasthenic patients closely, including respiratory function, when starting triamcinolone, particularly at high doses. Consider starting treatment in a monitored setting and adjust the pyridostigmine dose if weakness worsens.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
