Summary

Pyridostigmine increases acetylcholine by inhibiting cholinesterase, while trospium blocks muscarinic receptors. The drugs oppose each other, so trospium can mask the muscarinic adverse effects of pyridostigmine, such as cramps and salivation, and reduce its gastrointestinal and bladder effects. Pyridostigmine's nicotinic effect on muscle strength is not blocked.

What this interaction means in plain words

Pyridostigmine and Trospium can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Pyridostigmine belongs to Central Nervous System; Trospium belongs to Genitourinary.

Evidence level: Moderate

Why it happens

Pharmacodynamic antagonism between a cholinesterase inhibitor and a muscarinic antagonist.

What to do

Monitor for symptom control of myasthenia and for muscarinic effects. Be aware that cholinergic toxicity could be masked, and adjust doses as needed.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
