Summary
ACE inhibitors lower aldosterone production and spironolactone blocks its receptor, so together they markedly reduce potassium excretion. Severe hyperkalemia and acute kidney injury can result, particularly in older patients, diabetics and those with renal disease.
What this interaction means in plain words
Quinapril and Spironolactone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Combined suppression of aldosterone synthesis and receptor blockade reduces renal potassium excretion.
What to do
If combined (for example in heart failure), start with low spironolactone doses and check potassium and creatinine within the first week and regularly thereafter. Stop or reduce therapy if potassium rises above safe limits.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
