Summary

Non-acetylsalicylic salicylates such as salsalate or choline salicylate may weaken the blood pressure effect of ramipril and affect renal function, similar to other NSAIDs. The effect is probably smaller than with potent NSAIDs and is most relevant in dehydration or kidney disease.

What this interaction means in plain words

Ramipril and Salicylates can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Inhibition of prostaglandin synthesis reduces renal vasodilation and sodium excretion, opposing ACE inhibitor effects.

What to do

Monitor blood pressure, serum creatinine and potassium with regular or high-dose salicylate use, and keep patients well hydrated.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
