Summary
In patients with myasthenia gravis, corticosteroids can transiently worsen muscle weakness, and cholinesterase inhibitors may add to this concern. Rivastigmine is mainly used for dementia, so the interaction matters mostly in patients who also have myasthenia gravis.
What this interaction means in plain words
Rivastigmine and Triamcinolone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Rivastigmine belongs to Central Nervous System; Triamcinolone belongs to Endocrine.
Why it happens
Corticosteroids can cause early transient worsening of myasthenic weakness; the mechanism of the interaction with cholinesterase inhibitors is not well established.
What to do
In patients with myasthenia gravis, monitor muscle strength and respiratory function closely when starting triamcinolone. Reassess the cholinesterase inhibitor dose if weakness appears.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
