Summary

Salsalate is a salicylate NSAID with weak prostaglandin inhibition, but it may still reduce renal function and blunt the antihypertensive effect of sacubitril-valsartan. Hyperkalemia and acute kidney injury are possible, especially in older or volume-depleted patients.

What this interaction means in plain words

Sacubitril-valsartan and Salsalate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Inhibition of renal prostaglandin synthesis reduces renal perfusion and sodium excretion, compounding the renal effects of angiotensin receptor blockade.

What to do

Use the lowest effective dose for the shortest time and monitor creatinine, potassium and blood pressure. Consider paracetamol as an alternative.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
