Opioids with serotonergic activity combined with MAO inhibitors have been associated with serotonin syndrome. Sufentanil has weak serotonergic effects, but the risk with an MAO-B inhibitor cannot be excluded.

What this interaction means in plain words

Safinamide and Sufentanil sl can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Low

Why it happens

MAO inhibition combined with opioid serotonergic activity may raise serotonin levels; opioid interactions with MAOIs may also cause CNS excitation or depression.

What to do

Avoid the combination where possible and choose an alternative analgesic. If it must be used, monitor for agitation, hyperthermia, clonus and respiratory depression, especially at initiation.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
