Summary

Vilanterol is a long-acting beta2-agonist, and its cardiovascular effects such as tachycardia, palpitations and blood pressure changes can be amplified in patients taking MAO inhibitors. Selegiline at higher or non-selective doses, and especially the transdermal form, carries the greatest concern. The effect may persist for about two weeks after the MAO inhibitor is stopped.

What this interaction means in plain words

Selegiline and Umeclidinium bromide / vilanterol inhaled can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

MAO inhibition raises adrenergic tone and may potentiate beta2-agonist cardiovascular stimulation.

What to do

Monitor heart rate, blood pressure and ECG symptoms when starting or adjusting the inhaler. Consider a non-beta-agonist bronchodilator alternative if cardiovascular effects appear, especially with transdermal selegiline.
