Summary
Both drugs can affect cardiac conduction. Siponimod causes transient bradycardia and AV conduction delay at initiation, and trifluoperazine, a phenothiazine, can prolong the QT interval. Together they may raise the risk of arrhythmia, including torsades de pointes, particularly with bradycardia.
What this interaction means in plain words
Siponimod and Trifluoperazine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive pharmacodynamic effects: siponimod-induced bradycardia combined with phenothiazine-related QT prolongation.
What to do
Obtain a baseline ECG before starting siponimod and seek cardiology advice if the patient is on a QT-prolonging drug. Correct potassium and magnesium and monitor heart rate and ECG during initiation.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
