Summary
Siponimod reduces circulating lymphocytes, which may blunt the immune response to vaccination. A live attenuated vaccine such as yellow fever carries a risk of vaccine-related infection in patients with suppressed immunity.
What this interaction means in plain words
Siponimod and Yellow fever vaccine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Siponimod sequesters lymphocytes in lymphoid tissue, causing immunosuppression that impairs vaccine response and allows replication of live vaccine virus.
What to do
Avoid live vaccines during siponimod therapy and for about 4 weeks after stopping. Give any needed live vaccine at least 1 week before starting siponimod, and use non-live alternatives or a documented waiver where appropriate.
