Summary

Penicillins such as ticarcillin can chemically inactivate aminoglycosides in vitro, and in patients with severe renal impairment this may lower measured and effective streptomycin levels. The effect matters mainly when samples are stored with the penicillin or when clearance of both drugs is greatly reduced.

What this interaction means in plain words

Streptomycin and Ticarcillin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Nucleophilic opening of the beta-lactam ring inactivates the aminoglycoside, mainly in vitro or in renal failure where both drugs persist.

What to do

Give the two drugs at separate sites and times, and in renal failure monitor streptomycin levels and clinical response. Process serum samples promptly or freeze them to avoid in-tube inactivation.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
