Summary
Aminoglycosides can cause neuromuscular blockade, and magnesium at high systemic levels also impairs neuromuscular transmission. Together they may produce muscle weakness or respiratory depression, especially with parenteral magnesium or renal impairment. Oral magnesium citrate has limited absorption, so the risk is mainly in renal failure.
What this interaction means in plain words
Streptomycin and Trimagnesium can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive inhibition of presynaptic acetylcholine release and neuromuscular transmission.
What to do
Avoid unnecessary magnesium supplements or laxatives in patients with renal impairment receiving streptomycin. Monitor for muscle weakness and respiratory function, and check magnesium levels when relevant.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
