Summary
Tolmetin may reduce the blood pressure control from telmisartan. Combined with an ARB it can also cause renal function deterioration and hyperkalemia, especially in the elderly or volume-depleted.
What this interaction means in plain words
Telmisartan-amlodipine and Tolmetin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Telmisartan-amlodipine belongs to Cardiovascular; Tolmetin belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandin synthesis reduces renal perfusion and sodium excretion, opposing ARB action and promoting renal impairment and hyperkalemia.
What to do
Monitor blood pressure, serum creatinine and potassium, use the lowest effective NSAID dose for the shortest time, and maintain hydration.
What to ask your pharmacist
- Should I take them at different times of day?
