Summary
NSAIDs can reduce the antihypertensive effect of angiotensin receptor blockers and, together, may impair renal function, especially in elderly or volume-depleted patients. Hyperkalemia can also occur.
What this interaction means in plain words
Telmisartan and Tolmetin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Telmisartan belongs to Cardiovascular; Tolmetin belongs to Musculoskeletal.
Why it happens
NSAID inhibition of vasodilatory renal prostaglandins reduces glomerular perfusion, which combined with angiotensin receptor blockade lowers filtration and impairs potassium excretion.
What to do
Ensure adequate hydration and monitor blood pressure, serum creatinine and potassium after starting or changing the NSAID. Avoid in dehydrated or renally impaired patients if possible.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
