Summary

Xipamide is a thiazide-like diuretic that causes potassium and magnesium loss, while triamterene is potassium-sparing. Together they give complementary diuresis and the potassium-sparing effect offsets hypokalemia, but fluid and electrolyte shifts, hyponatremia and hypotension can still occur. Hyperkalemia is possible in renal impairment.

What this interaction means in plain words

Triamterene and Xipamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive diuretic effect from action at different nephron sites, with opposing effects on potassium handling.

What to do

Monitor serum potassium, sodium and creatinine within the first weeks of combined therapy and periodically thereafter. Reduce doses or stop one agent if electrolyte disturbance or volume depletion develops.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
