Summary

The oral live attenuated typhoid vaccine could cause disseminated infection in patients receiving an immunosuppressive biologic such as ustekinumab. Immune response to the vaccine may also be reduced.

What this interaction means in plain words

Typhoid vaccine live and Ustekinumab are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: Moderate

Why it happens

Immunosuppression from interleukin-12/23 inhibition impairs control of replicating attenuated vaccine organisms and may reduce vaccine immunogenicity.

What to do

Avoid live typhoid vaccine during ustekinumab therapy; use the inactivated injectable Vi polysaccharide vaccine instead. If a live vaccine is needed, give it before starting therapy, following the labeled interval.
