Methyldopa may lower blood pressure by stimulating central inhibitory alpha-adrenergic receptors, false neurotransmission, and/or reduction of plasma renin activity.

Starting at 16 to 20 weeks was linked to a slightly smaller head size, with no effects at follow-up. An option for high blood pressure in pregnancy; NICE prefers labetalol, then nifedipine.

- Safe Passes into milk in small amounts that are not expected to affect the infant; LactMed: no special precautions. US label: use with caution. NICE advises stopping methyldopa within 2 days after the birth and changing to another antihypertensive.
