Mécanisme d'action
Vonoprazan suppresses basal and stimulated gastric acid secretion by inhibiting the H+,K+-ATPase enzyme system in parietal cells in a potassium-competitive manner. It binds to active pumps noncovalently and reversibly and does not require activation by acid.

Healing of erosive esophagitis: 20 mg orally once daily for 8 weeks

Proton pump inhibitors are usually taken by mouth once a day, often before a meal, for a course whose length depends on the reason for treatment.
Grossesse et allaitement

- Unknown No adequate and well-controlled studies in pregnant women; available data are insufficient to evaluate drug-associated risk. A pregnancy exposure registry exists.
- Allaitement
- Unknown Vonoprazan is present in breast milk at less than 0.03% of the maternal dose. No data on effects on the breastfed child. Animal liver injury occurred in offspring. Weigh benefits of breastfeeding against maternal need.

Contre-indications
- Known hypersensitivity to vonoprazan or any component of the product (reactions have included anaphylactic shock)
- Concomitant use with rilpivirine-containing products
Précautions particulières
- Symptomatic response does not preclude gastric malignancy; consider further follow-up and testing, and endoscopy in older patients.
- Acute tubulointerstitial nephritis: discontinue and evaluate if suspected.
- May increase risk of Clostridioides difficile-associated diarrhea; use the shortest duration appropriate.
People taking proton pump inhibitors for a long time should have the need for treatment reviewed, because long-term use is linked to low magnesium, vitamin B12 deficiency, bone fractures and gut infections. Tell your pharmacist about all other medicines, since some interact.
