Résumé
Atenolol blocks the beta effects of epinephrine, leaving alpha-mediated vasoconstriction unopposed, which can cause marked hypertension with reflex bradycardia. Beta-blockers may also reduce the effectiveness of epinephrine in anaphylaxis. Chlorthalidone-induced hypokalemia may add to arrhythmia risk.
Ce que signifie cette interaction, en termes simples
Atenolol-chlorthalidone and Epinephrine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Atenolol-chlorthalidone belongs to Cardiovascular; Epinephrine belongs to Endocrine.
Pourquoi elle survient
Beta-1 blockade by atenolol antagonizes epinephrine's cardiac and bronchodilator effects while unopposed alpha stimulation increases vascular resistance.
Conduite à tenir
Monitor blood pressure and heart rate when epinephrine is given. In anaphylaxis unresponsive to epinephrine in a beta-blocked patient, consider glucagon.
