Résumé
Carbamazepine is a strong inducer of CYP3A4, CYP2B6, CYP1A2 and P-glycoprotein and accelerates the clearance of lorlatinib. The result is reduced lorlatinib exposure and loss of tumour control; strong inducers have also provoked marked transaminase elevation.
Ce que signifie cette interaction, en termes simples
Carbamazepine and Lorlatinib are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Carbamazepine belongs to Central Nervous System; Lorlatinib belongs to Oncology.
Pourquoi elle survient
Enzyme induction by carbamazepine increases metabolic clearance of lorlatinib
Conduite à tenir
Do not co-administer; use a non-inducing alternative to carbamazepine, and allow enzyme induction to wane (several weeks) before starting lorlatinib.
