Résumé

Rivastigmine, a cholinesterase inhibitor, may increase the risk of extrapyramidal symptoms when given with metoclopramide. Cholinesterase inhibitors can also worsen parkinsonism, and metoclopramide's dopamine blockade adds to this. Neuroleptic malignant syndrome is a rare concern.

Ce que signifie cette interaction, en termes simples

Metoclopramide and Rivastigmine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Metoclopramide belongs to Gastrointestinal Tract; Rivastigmine belongs to Central Nervous System.

Niveau de preuve: Moderate

Pourquoi elle survient

Metoclopramide blocks central D2 receptors while rivastigmine increases cholinergic tone, and the two together promote extrapyramidal effects; cholinergic effects on gut motility may also be additive.

Conduite à tenir

Avoid the combination where possible, especially in patients with Parkinson disease or dementia. If needed, use the lowest metoclopramide dose briefly and monitor for tremor, rigidity and dystonia.
