Ciprofloxacin markedly increases tizanidine exposure, with studies showing roughly a ten-fold rise in peak concentration and about a 10-fold rise in AUC. This can cause severe hypotension, bradycardia, marked sedation and dizziness. The combination is contraindicated in tizanidine labeling.

Ciprofloxacin and Tizanidine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Ciprofloxacin is a potent CYP1A2 inhibitor and blocks the main metabolic pathway of tizanidine, greatly increasing its systemic exposure.

Do not co-administer. Choose an antibiotic that does not inhibit CYP1A2, or temporarily replace tizanidine with another antispasticity agent such as baclofen.
